DNV Spain - MI-T and MI-F Forms

Hi all,


Hoping someone can provide some guidance on completion of forms MI-T and MI-F, for submitting when within Spain.  More general queries I guess, but just want to ensure they're accurately comoleted:



MI-T for DNV:


Section 1: international teleworker?


Section 2: initial and investor/teleworker visa holder?


Section 3:  should I add the address of where I'm staying in Spain while submitting the visa?


Section 4: My employer is in the UK, should I fill out all the sections I can with the UK office address?


Section 5:  our application is being submitted by proxy, by an individual on our behalf, should this section be their details?


Unsure what the tick box is at the bottom of the document, should this be checked.


MI-F for DNV:


Section 1: family member of international teleworker?


Section 2: initial and holder of investor/teleworker visa resident?


Section 3: should I add the address of where I'm staying in Spain while submitting the visa?


Section 4: details of the main applicant of the DNV?


Section 5:  our application is being submitted by proxy, by an individual on our behalf, should this section be their details?


If anyone has any experience on these forms, any guidance would be much appreciated.
