Not sure what a retirement Kitas is, but with a wife sponsored Kitas or Kitap even if neither of you are working you can get BPJS. Having said that, hospitals don't cover every illness, some give partial coverage and some full coverage but for most important illnesses I think you will get full coverage. And, you can only go to hospitals on the BPJS list for the illness that you have. After you have your Kitas be sure to get your KTP as that is important for BPJS.

Retirement kitas cannot get bpjs , even maried kitas or kitap have problems in some parts of indonesia

I can't speak for others, but for us, the whole thing was done by my wife and we got BPJS without any problems at all. There was never any question about me not being eligible. It's difficult to understand how some cities accept expats for BPJS and some don't, as I am sure the requirements are the same irrespective of where you live in Indonesia. Maybe some applicants aren't doing it the right way...
