S1 forms- medical cover in Spain

I am moving to El Campello in Alicante Province next week on February 6, 2024.

I received the S1 forms from the UK.

I have to submit these forms to the INSS office near me.

Does anyone know the address of the office where i must submit the S1 forms.

Are there any special requirements.
