The UK National Service (NHS) has what they call a "NHS Prevention Programme", it is available to all legal UK residents.  This link will take you to the specific page of the NHS website.  Like all such matters, the initial assessment is conducted at your local GP/doctor, but it will be a Healthcare Assistant who will gather the key information which is then assessed by your GP for future referal if required.
