I arranged health insurance for my partner thru status with Cosmos insurance. The application form asked for medical history and then after submission they called to ask more questions. The quote they gave initially was only based on age so of course it increased after the application. Overall the annual  price here was the same as I was paying Bupa in the UK each month!
