Almost Most of the tattoo artistin Saudi Arabia are underground.

As long as the person maintains hygienic standards then there shouldn't be an issue, whether it's underground or not.

Hi, I know one place, it's just behind the Jeddah International Market, along the old canal. You can even rent a bicycle using credit card (self service). All the best

I wouldn't assume any potential tattoo "shops" here in the KSA are going to be properly licensed, therefore they won't be monitored and would most likely lack cleanliness and health-safety issues that you may encounter. 

In short, wait for your vacation, book a flight to a tattoo-friendly place and get it at the tail end of your trip (so you can swim for most of it)
