Physical Therapist who can home visit in CM?

A good friend of mine (Male in his 80's) has just been discharged from hospital in CM and could really benefit from a good physical therapist (who is reasonable but good). Any suggestions as to a person or office that has good reputation. He is rehabbing a leg.
