I have an imminent appointment with Paphos Immigration in order to start the paper MEU3 to biometric card process. I have spent a while online trying to find the application form.

I know this must be easy but it's the end of a long day and I'm reaching dead ends.

Would appreciate a link plus I then need to visit a printer as mine is playing up so recommendation appreciated.
