I don't know much about your field, but I don't think that it will be easy for you to work here unless you can communicate in LV and RU.
There are two reasons that I would say this:
1. Communication is an obvious req. In Your field;
2. There are many medical graduates in the country, who went through school on Government money, and are paying off there 'loans' with obligatory service. There are plenty of poorly paid doctor office and hospital workers in the public system.
There is a signifcant private health care system, but I don't think that there are any private ICUs.
You could consder writing to the ARS medical center for advice. They are a popular privat clinic that have english speaking staff (I'm a foreigner, and it's where I go for service.) The may be able to offer advice.
