It can happen anywhere.

Oh, my gosh! Someone had to have gotten hurt in that one. Very stupid and careless!

Very stupid indeed, I doubt the driver survived, he hit the truck very hard and the car behind him also hit him and hit him hard. Either way he will not be trying that again,
That is the best example of an accident I could find and I seen that mistake (without the accident) occur many times.
