Why does the immigration cancel the visa of person with same name even if they have different passport number

Too little information.

Could be a simple admin error or maybe they suspect someone is trying to enter after being blacklisted but the offender has made a new passport in an attempt to get around a ban.

If you made new passport ... It doesn't mean that your old passport is disappeared from planet.
Your old passport number is mentioned on your New passport as "Previous passport Number" and it can easily b traceable with the help of your country ID number and many other ways.
