Form to avoid the ~30% tax on importing personal/used items

Is there a form we would need to fill out to avoid the ~30% tax when importing personal (used) items into Colombia?  We are sending just a few boxes.

I thought I read somewhere that if a Colombian (my spouse) was outside of the country for a certain period of time, she would not have to pay the tax on certain items if brought into the country.

She looked at the DIAN site, but couldn't find anything.

These links should help.  If I remember correctly the tax exemption applies if she's returning after at least 5 years (although one link says 3 years), for various items in your menaje de casa:

...and other similar searches.  I don't know if there's a form involved - but knowing the Colombian bureaucracy, there almost certainly is at least one form required...
