First go and read this:

Then log into the EMS system and click on New on the left side. Choose new expatriate permit and fill in the fields. On the drop down, choose dependent visa. Attach the required documents in JPG format and with size of less than 200 KB. Submit. And then wait. If any issue, they will let you know.

Every passport number is a separate application.

It is a straightforward system. If you can't figure out how to use it then get someone else to do it for you. You need to be comfortable with computers. When I used it for the first time ever to apply for visas; it took me all of 5 minutes per application. And most of that time was spent in resizing the scanned images to get the system to accept them.

Or you go to LMRA counter and apply in person. They will do exactly the same process that you can do at home.
