My wife is an O&G in the public healthcare system.

Only citizens get subsidized healthcare. non citizens must pay full fee. However, this full fee is still cheaper than going to private healthcare.

Public vs Private? Well the caveat here is, you have to wait your turn at public hospitals. This may take couple of hours. Or possibly half a day. And, public healthcare fees are significantly cheaper than private even for non citizens. Most famous health specialists spend their time between public and private, maybe 1 or 2 days for a couple of hours a week at a private hospital.

No, Malaysia does not have social security.

If you don't want to wait at a public hospital. make sure you pay for medical insurance so you could go straight to a private hospital.
