Hello everyone!

I bought something from Japanese Amazon and then entrusted a transport agency to pack my things and sent them to Madrid. Now the package is held in customs and according to a letter I received, I filled "Documento de tramitación con Correos" out and submitted all the necessary papers on ADTPOSTALES.COM.

However, today I saw that the "Documento de tramitación con Correos" I submitted has been rejected, and meanwhile, I received "Email Peticion Documentacion", which said:

So what's going on with the documents I submitted? Seems that there're some problems with the part 1 and the part 5, but I don't think I write something wrong in the part 1 (except that I only have one surname but the customs or the Correos guys insist that I should and I must have the second one, so I repeated my only surname for Segundo Apellido), and for the part 5, I'm sure I had listed every single thing in that package with its price according to the Amazon's receipts(except the shipping and handling fee... so they want that the prices' sum should include the shipping and handling fee? If so, how could I declare the shipping fee's importation?!). And besides, what's the "escrito firmado explicando si es la suma de los distintos justificantes de compra que adjunta"?

Help me, please!

Thank you very much!
