I found this in the website of Ministry of Interior

Procedures for adding people born outside the Kingdom:

1. Pay the entry visa fee of 2000 Saudi riyals through the automated payment system.

2. Medical insurance

3. Fill out the application form with the child's name + two photos. (6×4)

4. Addition should takes place within 3 months according to the entry stamp.

5. If the addition is not made after 3 months, he will be fined under Article 55 of the Residency Law.

6. The newborn must be inside the Kingdom during the addition.

7. The newborn must have an independent passport.

8. The parents must reside in the Kingdom.

9. The wife must be sponsored by the husband.
