I cannot get my brain to understand

400,000 inpatient and 40,000 outpatient with 100,000 deductible.

How does 100,000 deductible work with 40,000 outpatients?

i know dumb farang.!

The deductible will spread over out and inpatient, using the deductible where the expenses are made. Until 100.000 is reached

You need to check if an increase in premium is worth the reduction in deductible

That should be possible with thai insurers.
