Assuming that you hold Irish citizenship:
1.You should check with the folks at SSI about what will happen if you move abroad. There is an American Embassy in Dublin that you can liaise with while living here if you need to be periodically assessed.
2. Unless you have lived and worked in Ireland before (and accrued a certain amount of PRSI contributions) you won't be eligible to collect any Social Welfare payments. Those days are over.
3. If you have an adequate number of PRSI contributions, there is a bilateral Social Security agreement between the US and Ireland, although as far I know you would still have to satisfy a Habitual Residency Condition.

First of all, you will not be able to collect any disability payments from Ireland's social services unless you are an Irish citizen. You may also have to establish something called Habitual Residency, which takes around two years. You will, of course still be collecting your disability from your country of origin.
