Taking a pet to Malaysia

Hello people of Malaysia :)

I am currently writing the EB guide about Malaysia and I need some precision about pet importation.
I read that you need to produce an "affidavit of the captain of the airline" for pets entering from the United Kingdom, Eire, Northern Ireland, New Zealand, Japan and Brunei. I mean... what's that "affidavit of the captain of the airline"?

If any of you knows more about pet importation in Malaysia please post below :) It would be of great help for me and for pet owners on their way to Malaysia :)
