Suitable place to settle and work

I am a newbie to the Belize forum.  I am a retired doctor and lawyer and am planning a move to a warm  area OUT OF THE US at the first opportunity (e.g. when my teenagers are 18 and my parents are dead.)
One attraction to me of Belize is the purported lack of formality in business, because I would like to open a little practice doing acupuncture but I don't want to have to get a license, let alone get a doctor license, in Belize.  I am wondering if anyone knows anything about whether I can do acupuncture in belize without having to get a license.
The areas that attract me in Belize are the Cayo district (rivers, waterfalls) and Ambergris Caye (ocean, reef).  I really know very little more about Belize and I really don't have an idea of where to look for a place to settle.  I like to be around water obviously, but I also like good internet connection and easy travel back to the US.  I also like to be around other expats, but not to the exclusion of learning to live with local people.  So I guess it boils down to this: I don't want to live in an expat "gated community", I would like to live near the ocean or a river system, and I need fairly good insfrastructure.
One more thing:  I was in Ambergris Caye many years ago and the mosquitos were TERRIBLE.  I hate mosquitos and if any place has an abundance of these critters, then that would probably rule it out.  That said, I would still like to hear about anyplace, mosquitos or not.  Just mention the mosquito situation though.  And yes, I know that mosquitos come with water.

Sorry you cannot work at all in Belize without a work permit not easily obtained and if it is anything that would need a licence elsewhere in the world would need a licence also in Belize. You have been slightly misinformed if you have the impression there is lack of formality in Business in Belize as a foreigner you will find Belize LOVES its red tape.

you can retire there, you can employee people there but you cannot work there. mosquitos and tropical go together so that leaves out central america and most of mexico and most of S. America for you.

I guess my home of Cayman Islands may be the only place in the Caribbean and Central America that has a pretty comprehensive government-run mosquito-control program. It doesn't work 100%, but it has eliminated the aedes aegypti mosquito, and so we have no endemic malaria or dengue fever, or anything similar - or zika, of course.
