Well I just returned from my visa renewal here in México City, and I hope this will be useful. (You can renew your visa 30 days before it expires.)
1)     Fill out and print this form, make sure you turn of all pop-ups.
Solicitud de Estancia - Instituto Nacional de Migración
2)    Fill out and print this form (you can go to INM and pick up this form and they will give you the instructions on how to fill it out, or you can just print this one.)

3)    Fill out and print this form then you can take this to any bank and pay $,1294 also make a copy of the receipt for your records. INM will keep the original.

4)    If you are using your bank statements for proof of income, do not bring any part of your statement that does not pertain to your account. My bank prints information about new bank policies in the top portion of my statement and I had to have that information translated into Spanish on a separate paper. Also as a tip use a highlighter to highlight your deposits and balance. Trust me this will help greatly.
5)    You will need a cover letter. This is an example of the one I used I received this info from INM

6)    You will need 1 black and white copy of your passport (the I.D. page only)
7)    Bring your old visa
8)    Get 5 infantile photos 2.5x3.0 they are smaller than the old ones and can be in color. 3 of the front and 2 of your right profile
9)    I did not need it but just in case bring a copy and original of a utility bill
10)    Take all of your documents to the INM office.   When you walk in  you are facing the information counter, go to the first line to the left and wait to be called. (Try to get the older lady with blond hair and glasses, she speaks a little English.):D
In one week you will return to INM to receive your new visa.

Careful, this may have applied to Mexico City only in 2009.  In September of 2010, in Zacatecas, the procedure varied quite a bit.

No matter what any government of private website might say, the safe bet is to call or go to the local INM office and ask what the current requirements are.

In Zacatecas, I first had to turn in the current application and other paperwork required.  I probably could have gotten the photos taken ahead of time but decided to wait to see if they told me something different when I went.  I got the photos taken and when I turned them in the gave me a computer printed form listing the fees I needed to pay and the file number of my case and account number into which the money had to be deposited.  I was told that although the deposit could be made at any bank, going to Banamex made things easier.

We live in the center of Zacatecas and branches of all the banks are within a two block stretch on the main drag, so why not.  I returned with the deposit receipt and that was that.

Given that the official deposit ticket appears to be required now, I doubt you can get it done in less than two trips to the INM office.

Renewed FM-3 in March 2011. Never been that smooth.
Once the Form (ad)1) Silence mentioned is filled in, send it online. You will get a file number to track status of your application for renewal online. When you get the info to present your files at the Migra office, bring this form, the fotos mentioned, proof of income and get your "formato de pago" for reneal fees (1294 Peses), deposit the fees at the bank, bring the confirmation back to the Migra office and hand it all in. Got my new "tarjeta de residencia" 10 days later.
I live in the Bahía de Banderas, north of Puerto Vallarta and my Migra office is in Bucerías. All staff in this office speak good English, are very friendly and give any assistance needed.
