About the visa, I've got a J-1 visa for 6 months, and my company is getting me a 3 years H-1b visa. So no, it is not difficult if a company is sponsoring you. The procedures and the amount of forms to fill in is quite impressive but it is easy.
Otherwise, when you are not sponsored, it is clear that US Immigration services will be far more picky about who you are, what you want to do on US soil etc. I would say that I don't think it is possible to get a work visa without being sponsored by a US company. Last chance is to subscribe to the greencard lottery!
