Requirements :

To become Indonesian

1. Copy of Birth Certificate which has been translated into Bahasa
2. Photocopy of birth certificate of the applicant's husband or wife
3. Copy of identity card of the applicant's husband / wife
4. Photocopy of Marriage Deed of the book marriage of applicant husband or wife( translated in bahasa,optional)
5.Original certificate from the Immigration Office where applicant resides (SKIM)
6.Original certificate of Kepotisian (SKCK)
7. Five years continuous legal living in Indonesia or ten years on and off whilst holding a KITAS or KITAP Original certificate from the applicant.
8. Latest photo of applicant colored,with a red background
9. Original certificate of physical and spiritual health issued from the hospital.
10. Original proof of payment PNBP.
11. Photo 4X6 cm,4 pieces
11. Letter of statement.

As a note, the text in the OP makes the process sound easy, but it isn't.
There's a lot more to it than that.

Number 11 is wrong; there are 4 letters, all must be hand written and signed over a stamp.
