Employment visa

What is your nationality?   that is what decides everything.  Being born in the GCC doesn't matter at all if you are an expat.   

The only nationalities that can work anywhere in the GCC without sponsorship are GCC nationals.  No one else.   For everyone else, you either work for an employer who sponsors a visa for you or set up your own business.

Saudi

If you are Saudi, you don't need an employment visa but you do need an employer.  That employer will put you on his or her payroll and complete the necessary formalities.

Will I need to do the medical exam

No. At least not until a few years ago. Don't think that's changed now.

What about my friend. He is a us citizen but is Syrian and lived and went to school in saudi

Needs employment visa and will go through medical.
