Health Card - how I get it? HELP!!!

Hi - I moved to Milano 5 months ago and today just got my Permesso di Soggiorno. The next step now is to obtain my health care card so I can access the Public Health System. What is next to do? I live in Milano so please let me know where exactly I have to go and what should I bring with me.
