It seems that the problem that prevented those without a NIK from registering on the app has finally been fixed.
Below is a step-by-step guide to registering, published by Jakie Pommeroy on his Bali Covid19 uodate.

PeduliLindungi Registration Guide

So you:
- Are on B211 without NIK
- Got at least one jab in Indonesia
- Got that non-official looking paper proof "KARTU VAKSINASI COVID-19"
- Have the PeduliLindungi app on your handphone and have done the basic registration process.
If possible do this on your laptop so you can save some images more easy.
- Go to https://vaksinln.dto.kemkes.go.id
- Don't worry, this whole process is in English
- Register an account. You'll get an automatic confirmation mail seconds later. Log in.
- Fill in your data and upload a pic of your passport and scan/picture of your paper proof(s)
- Submit and wait.
- Wait. You can go any time to https://vaksinln.dto.kemkes.go.id and it should show your profile
and tell you it's in verification process.

- After 15 days I got a mail from "Kementerian Kesehatan RI" with the subject "Vaccine
Verification Result"
- It reads "Hi YOURNAME, Congrats! Your vaccination verification request is completely
approved."
- Click the link and log in again. Nothing much to see there though.

I did the following process in less than 5 minutes. Everything should be real-time. There should
be no waiting for emails etc.
- Go to https://www.pedulilindungi.id/periksa-sertifikat
- Log in with the email you used for your PeduliLindungi app
- You'll get a code in your email. Insert it on the web page
- This page is in Indonesian:
Periksa Sertifikat Vaksinasi COVID-19
Nama Lengkap ---> Full official name
NIK / Kode Negara + No. Paspor ---> your 3-character country code and your full passport
number (eg. USAXY1234ABC567)
Tanggal lahir ---> Your date of birth YYYY MM DD
Tanggal Vaksin ---> Your jab date as on the paperwork
Jenis Vaksin ---> Select "Vaksin Luar Negeri" That means "Vaccinated abroad". Do NOT put
AstraZeneca or whatever as it won't work.
- RE-CHECK your data, especially whether your birthdate is correct.
- Select I'm not a robot and click the "PERIKSA" button
- Wait. Can take a while.

- On top of the page at the right click on your name and select "Akun Saja".

- Click "Perbarui Profil" on the right
- Make sure
Identitas Akun ---> "Warga Negara Asing" - that's a "foreigner"
Negara Penerbit Paspor ----> Your country
Nomor Ponsel ---> Handphone number in +6281xxxxxxx format
Tanggal Lahir ---> Date of birth
- Click on "Simpan" which means "Submit"
- Go to your PeduliLindungi app on your phone
- Click "Paspor Digital" - 3rd icon on the bottom of the page
- Click your name
- Your certificate should be visible now.
