I am going to work in public hospital in sakaka who pay for my iqama?

Normally  we are told that the hospital pay the Iqama

Your employer is responsible to pay and process your work permit (Iqama), within 90 days, upon your arrival in the Kingdom of Saudi Arabia.

YES definitely. Your employer (Public Hospital) are responsible for the processing, and issuance of your work permit (iqama), as well as renewal of your iqama to work and live legally as expatriate in Saudi Arabia.

The current system of my employer is not working...is someone knows that my current employer helps me to transfer aqama through jawazat???

Yes, he can do that manually through jawazat.
