Found some old microscope slides and doctor's notes in bulgarian

Hello everyone!

I was born in Bulgaria and we moved to Canada when I was 7 and I haven't been back.

While cleaning out my mom's old things, my sister found some old microscope slides and a few doctors notes on them.

We're not native Bulgarian speakers and the only thing I can translate on the notes is my name and the year.

I was wondering if someone would be kind enough to help me translate what the notes say. (My mom is alive and well but she says she doesn't remember what they were for and can't read the doctor's handwriting.)

I would prefer if we could sort this out in private, in case the notes contain any senstive information. Any help would be appreciated!
