I want to take CPR card for my wife...what all documents required for it??? i have taken appointment but confused on documents to carry

Go to Bahrain.bh. It lists all documents needed in the identity card section. But generally passports, proof of address (municipality letter or EWA bill), marriage certificates (attested) etc.

@XTang marriage certificate attestation is mandatory?? some are saying not necessary some say its mandatory

Yes mandatory. If you don't get asked for attestation, then its your luck.
