For expats, care of elderly

Several toughts going my mind if getting older (my wife is 8 years younger) and I smoke, drink and do not sport (and my family die young).

OK I can hardly walk (so my wife is on her own) and I have some other issues (regular epilepsy is one of them), also incontencia from time to time.

What do you do? (do you hire a full time nurse?, do you expect your wife to take the burden, you try to go through health insurance or do you go back to your home country (in our case Netherlands). also far not from perfect, but then again with getting older that is so with almost all countries.

In Turkey family takes care of you (in any case that is what expected), but as an expat retiree unlikely to have family here (except if you are roots here)

OK I can wait for a few more years, but still thoughts.
