eea family permit as extended family member

this is my second question regading eea family permit as extended family member of eu national (italian citizens).I live in italy. I want to travel uk with my uncle (italian national).as extended family member of eu national am I eligible for for eea family permit as extended family member of eu national . My uncle Eu national (Italian citizen) is brother of my mother so what evidence should I present to show my relationship to my uncle. 1). I have birth certificate of my.my mother and my uncle . 2). We live in same house in italy and we have tenancy agreement with my and my uncle name. Is this enough proof for eea family permit as extended family member or I have to show other documents. Awating for your reply.

What is the legal requirement on the website? I'm sure that the EEA website will have the necessary information you need.
