I'm assuming that you mean a  VIVIS visa - a Visitor's Visa, valid for 90 days, with a possibility of one extension, at the discretion of the Federal Police.
The only place to go for an extension is the Federal Police.  It sounds like you may have already received the one extension you're allowed, but if they only gave you 45 more days, you could go back before your current extension expires and ask for 45 more.
