UK nationals retirement/residency in Malta

Unable to find route to residency and health care. Joint retirement income 40k/year.
Always end up being told to either transfer healthcare from UK using S1 or take out private healthcare before formal residency can be considered/completed. Is there any other route to residency?

You can use the RHA agreement for health cover but whilst that is sufficient for residency it does not give you the comprehensive health cover that the Certificate of Health Entitlement does (obtained via the S1 route).
The other thing of course is that no one knows for certain what happens in 2 months time if there is a 'No Deal' Brexit, either to the alternative ways of health cover (the S1 is an EU wide agreement) or even applying for Residency at all!

Your profile seems a little misleading as you say you have been in Buggiba since Feb. 2017 but currently live in Cambodia!
If the former is correct than you have already broken the law in Malta by not applying for Residency after being here for three months.

I am renting in Malta but spend most of my time in the UK and Ireland. I indicated based in Malta for this website.

The 1975 health agreement with Malta referred to is the RHA, which whilst OK for residence purposes does not give full coverage. The Certificate of Health Entitlement issued through the S1 is an EU wide agreement which could cease if there is a 'No Deal' Brexit
