Correct form to bring in my dogs

Hello,
Could someone please direct me to a link for the correct forms to have completed by my vet to bring my dogs on the airplane when I move to the DR from Canada.  Finding conflicting information.

Download and fill out the Trilingual form, then you don't have to have anything translated.  Your vet fills it out, and then you have to visit your closest Canada Food Inspection Agency to have it authorized by the Federal Government.  The page also has a link at the bottom to help you find your closest office.

Thank you so much.  This is the form I thought to use also but someone in customs shipping made it sound much more complicated as they were going to charge us extra to deal with paperwork there.  I was very confused as the dogs are coming with us on the plane. 
Did you use this form to bring your pets with no issues?

Also check with your airline - some have additional restrictions and forms to complete. We flew with American and they required the vet to sign a document stating our dog was not brachycephalic (short snouted).

And there are restrictions on temperatures - when are you planning to travel?

We are travelling November hopefully so I think we are good.  My 2 little ones will be under my seat (they are only 6 and 7 lbs).  The other 2 will checked in as they are about 15-18 lbs and Westjet under seats are smaller :-(

We did.  Both in separate kennels in cargo, and I taped a ziploc bag on top of the kennels with a copy of the form, vet check up and rabies certificate, just to make sure that whoever needed or wanted information would have it, rather than deny the dogs boarding.  Not sure if it made a difference, but it eliminated a source of anxiety for us, as we were moving permanently, had never flown with our dogs before, and one of them is extremely vocal and makes some horrible, unique noises for a dog.  I even went so far as to use a marker and write a disclaimer on his kennel, assuring that despite the noises he was making, he was truly fine.

Good point Uncle Buck! We also wrote our last name and phone numbers (US and DR) on masking tape on all 4 sides of the crate. A friend here brought in his three greyhounds and was delayed in being able to pick them up because the paperwork fell off one of the crates and they claimed they didn't know how to reach him. They didn't bother checking the other two crates for contact info!
