The insurance is only needed fr the Non O etc application at the embassy or consulate outside Thailand. And not by all of them.

It became a requirement during C19 but some have kept it in

For your extension there is no need.

Many insurance Companies offer such term insurances for a lower and agreeable price. Google C19 insurance.

In Thailand it is better to have an insurance... I have seen many foreigner go back broke or set up crowd funding for their bills.

Thai companies offer insurance that are affordable but do not cover existing issues.

Foreign companies do but at a premium price

Yes, do a Non-O instead.

@rbakker non O is 90 days. You have to then have to extend for 1 year ie OA and you must have health insurance right?

Wrong! No insurance needed for non-immigrant O. It remains as non-immigrant O even when you extend it for a year.
