You may find that there are not a lot of people that can answer this since it is outside the ordinary expat experience. Since that may be true, I will venture a guess that may or may not be valuable to you.

In the larger cities, there are a number of foreign-trained medical personnel. Your experience will probably be valued and so job opportunities may be available. You may even qualify for a Professional Visa. Three things to consider. First, you will need to speak Spanish fluently and the salary will be at Ecuadorian levels: that is, pretty low. Finally, the way things are done here are different than you are used to. You will have some adjustment to fit in and may find some conditions not to your liking. But, once the initial adjustment phase is settled, I think, after reading your profile, that you will find life better here.
