S pass is about to expire in less than a month but accdg to hr they are requesting for an extension.

Is this possible?

First time I heard this in 12yrs working in Sg.

TIA

Of course work passes can be extended.
If you worked here for 12 years already, you must certainly have encountered extensions yourself - or was your original pass valid fo 12 years???

Extension means renewal of your pass with the same employer. It’s subject to approval at MoM. It doesn’t give you an automatic extension.

How come you are here for 12yrs and you are not heard of work pass extension with an existing employer? Good luck

What confuse me is hr will request to Mom to extend it in one month.

Is there any penalty or conditions to be incurred?

Work passes are usually extended by a year, sometimes more.
It is unusual, but I don't see why it shouldn't be extended by a shorter duration if the employer requests this (e.g. because the employment ends then).
